The Modern Family actress, Sarah Hyland recently opened up about one of her lowest moments  while participating in the “post a pic of” trend on Instagram. On Monday, she shared a photo revealing about "a moment where you felt your worst but had to fake being okay for a pic."

In the photo, the 30-year-old Hyland can be seen smiling and sitting next to her friend Vanessa Hudgens at a 2018's event. “Was on dialysis and had my heart broken just days before this picture,” she wrote on her Instagram Story. She further wrote, "Thank god I had Vanessa Hudgens by my side at the event."

Fans pointed out that Hyland was referring to the time when she had a breakup with Dominic Sherwood in 2017 following her five-year relationship with her longtime boyfriend, Matt Prokop. Sources, however, revealed to E! News that the separation was "mutual and amicable and they truly remain friends."

The actress also posted another picture from her hospital bed, back in December 2018, when she underwent dialysis following a kidney transplant. Previously, she had opened up about her lifelong battle with kidney dysplasia. She also had revealed how after the first transplant her body rejected the new kidney, which her father had donated, and that led to the second transplant surgery. “I was very depressed,” Sarah had told Self in an interview in 2018. “When a family member gives you a second chance at life, and it fails, it almost feels like it’s your fault. It’s not. But it does.”

Hyland's fiancé Wells Adams has been supporting her through her various health issues. "Wells is always a supportive and loving boyfriend," a source told E! News in April 2019, "but he has been a particularly incredible source of support for Sarah during her recent health struggles." The couple has been dating for three years.

The actress had previously tweeted, “For those who are chronically ill and in chronic pain: Have you had the experience of doctors not listening to you? If so, how do you not tear their heads off with your bare hands?”
